Primarily Chronic Kidney Disease (CKD) from various causes (like glomerulonephritis, diabetic nephropathy). Also covers nephrotic syndrome and complications requiring hospitalization.
Medical evidence over ~90 days showing kidney function (like eGFR levels from blood tests). CKD: Requires very low kidney function, needing dialysis, or having serious complications (like bone disease, nerve damage, fluid overload, severe weight loss). Dialysis/Transplant: Being on chronic dialysis or having a kidney transplant automatically qualifies (transplant for 1 year). Nephrotic Syndrome: Requires specific lab results (protein in urine, low albumin in blood) plus persistent swelling.
